I set the my account page in the woocommerce settings.

You can try add the Account(icon) into Header Right Config in Theme Options Page.

The issue is resolved with Twenty16 theme.

Hi,
I have the latest Woocommerce Plugin(v2.5.5).
So, I tried to deactivate, delete and then reinstall it but My Account menu is still gone.

My site has the same issue after updating theme Version: 2.0.0
For Theme Version: 2.0.0 and Version: 2.1.1, My Account menu on the left side is gone.
But Version: 1.9.81 is fine.
