    Hi,

    The map was visible and working on the contact form though it seems to have disappeared even though the settings are all filled in in the shortcode. Suggestions?

    Hi

    You shouldn’t add the map in the contact form, you should add it in the page builder with the map asset

    Sorry, I’ve added it into the page builder “maps” 1/1 then under this have used the text block to add in the code of the contact form.

    Try disabling Contact Form 7 – seems that you have an issue with this here, it’s causing a jQuery error (nothing to do with the theme).

    I’ve used contact form 7 on other pages to create a custom form:

    By disabling contact form 7, will this remove these two above forms as well?

    @gorjess – if you disable, does the issue go away?

    If so, you’ll need to take the issue up with Contact Form 7 support – http://wordpress.org/support/plugin/contact-form-7
