Hi,

1) To add the cart: Theme Options => Header Options. Then drag in the Cart/Wishlist module.

2) Your contact form: you set the email address to receive the contact form submission from with the plugin, for each form.

3) When pasting in the shortcodes, be sure you first switch to the “Text” tab view and not “Visual”. Paste in your shortcodes, then activate the Swift Page Builder. Please see this:

Can you provide a screenshot of what you refer to? Do you mean the header cart as in my screencast?
