Hi,

You can use contact form 7 plugin and create the form. Then grab that form shortcode.

If you our page builder add a Text Block and edit it. Then using the shortcode generator that is in the editor toolbar add a shortcode, choose the modal and their settings. And place the previous contact form shortcode in the modal content.
